It has always been difficult to treat patients with neck problems related to working overhead. Interventions would prove beneficial, however, continued working conditions involving repetitive stress to the neck would require constant attention.

I wish that a neck support like this would have been available earlier during my many years of practice. I am sure it would have help prevent the long term consequences of continued neck compression often resulting in early degenerative changes in the disc and posterior vertebral joints, shortening of the neck muscles which are resistant to neck pain treatment and decreased productivity at work.

Pain From Working Overhead

Because neck and shoulder pain is a common problem for those who work overhead with the arms elevated above the shoulders, a 2015 study in the journal Work examined the effects of working overhead in relation to height and distance. They found both height and distance of overhead work were problematic, however, height caused more increased activity of the neck and shoulder muscles than distance.

The authors used electromyography to test the muscles involved; the deltoid, anterior serratus, trapezius and sternocleidomastoid. These muscles are frequently a cause of neck and shoulder pain and working in overhead postures significantly increases the activity of these muscles.

A 2014 study in the journal Ergonomics evaluated a wearable assistive device for overhead work. The device reduced perceived discomfort by 50% and was confirmed by electromyography

Overhead work (working with arms at or above the shoulder) places complex strains and stresses on tissues in the neck and upper extremities. This can result in impaired circulation and compression of tendons. This type of work is often found in industries involving tasks like attaching components to a vehicle underbody, drilling to attach anchor bolts and applying fasteners inside an aircraft fuselage, and other situations requiring overhead drilling in construction. Among several positive benefits, the device facilitated less extreme upper extremity working postures and reduced physical demands.

A 2012 study in the Journal of Physical Therapy Science found a significant increase in the trapezius and sternocleidomastoid muscles when working overhead with the head in extension. They found overhead work to be a risk for musculoskeletal injuries, also documenting increased strain on the rotator cuff of the shoulders. They also found increased strain using both hands overhead as compared to one.

A 2012 study in Industrial Health found that cervical range of motion decreased significantly with a short period of overhead work from tightness around the neck and shoulder muscles. Clinically reliable, cervical motion range is considered the most sensitive measure of the risk of neck pain.

A 2023 study in Human Factors found external support produced significant delay in fatigue onset in sternocleidomastoid muscles during overhead work and using an ergonomic intervention support can subsequently be used to alleviate postural demands and reduce the risk of musculoskeletal disorders.
