Most people sleep in the side position and this preference increases as age advances along with increased periods of postural immobility, which lasts between 45 and 110 minutes. With a trend towards increased immobility, choice of pillows becomes more important to avoid the negative effects of poor posture. While increasing periods of immobility are noted, adults tend to shift positions 11 to 13 times per night. It is therefore important to use pillows which not only support the neck in proper posture, but also accomodate changing positions.

Poor neck posture during the night effects pain sensitive structures of the neck and may be responsible for producing neck pain and stiffness upon waking, headaches and shoulder and/or arm pain. Neck related headaches are related to the upper part of the neck, while shoulder and arm pain can be referred from the lower levels of the neck; so pillows should provide support to all levels of the neck. Accordingly, pillow position may be related to specific areas as headaches may indicate rotational torsion from pillow which do not provide the correct surface for neutral positioning in the coronal or left and right rotational planes, while shoulder and/or arm pain may indicate pillows that lack a level support in sagittal or front and back planes. Neck pillows should also provide support in the left and right flexion plane which can relate to shoulder and arm pain and/or numbness. While in the side position with the left side up, pillows that are too high may cause undue stretching of the right neck and shoulder muscles, while compressing nerves and/or blood vessels of the left side of the neck.

This position corresponds substantially to the most anatomically natural position of the cervical spine of a person who is standing upright with correct posture.
