Neck Pain Disability

This is a neck functional disability questionnaire and can be used to evaluate the disability experienced by those with neck pain. The answers can be monitored over time to evaluate the course of neck disability and response to any type of interventions or treatments. It is particularly useful for long term monitoring of chronic neck pain.

Disability due to neck pain must be documented with objective findings: x-rays, mri findings, orthopedic and neurological tests ect... The questionnaire should be filled out and graded by your doctor to document your subjective findings. The combination of all findings form your overall disability regarding your neck pain. The purpose of this form is to give you an idea of the subjective qualifications for neck pain disability.

The neck pain disability consists of 15 questions relating to activity, pain levels, headache, ability to sleep, concentration, work and leisure activities and psychosocial factors.
